Process for Submitting Student Medical Forms

Medicals for New Students

All students being enrolled for the first time are required to complete and submit a medical either in-person to the Medical Centre, Papine Campus or electronically at


Western Campus Nursing Station or   (for Western Campus students).


You will be given a receipt after in-person submission of the medical which is to be taken to the Safety and Security Department to obtain your student identification card.  Electronic submissions will generate a message to guide completion of your student identification card.


Completing Medicals at the Papine Campus


Medicals can be completed at the UTech Jamaica Medical Centre, Papine Campus by appointment at a cost.  The student is to arrive at least 30 minutes prior to the scheduled time. Laboratory test can be performed at the onsite laboratory for an additional cost.

Medicals cannot be completed at the Western Campus.


Process for the Electronic Submission of Student Medical Form


Prospective Students do not have to visit the Medical Centre to submit medicals that were completed at health facilities external to the Campus.

 The process for the electronic submission requires the student to:

  1. Fully complete Part 1 of the Medical Form
  2.  Schedule an appointment at your medical doctor/clinic to fully complete Parts 2 & 3 of the Medical Form. Ensure that the Form is signed and stamped by the medical doctor/clinic.
  3. Obtain a copy of the result of the blood test and any other diagnostic investigation as deemed appropriate by the medical doctor.
  4. Have your Immunization Card assessed to determine if the vaccine requirements have been met.
  5. Obtain booster dose(s) of vaccine(s) to meet requirements.

Having completed steps 1-5 above, submit the following documents to either /

  • Completed Student Medical Form
  • Diagnostic Test results
  • Immunization Card

Receipt of the electronic mail will be acknowledged within 7 working days.
